ACHC-accredited Home Health Care Agency is expanding and accepting RN applications to join our team.

General RN Responsibilities:

1) Perform the initial patient assessment for start of care, (OASIS) using Kinnser/Wellsky software

2) Ensure all necessary paperwork is explained to the patient and signed

3) Generate the nursing care plan /aide care plan for the patient. Encouraging the patient/responsible party to participate in creating their care plan

4) Record and reconcile the patient's medications in the patient profile each visit

5) Notify the appropriate care team member if the patient is not receiving the proper level of care.

6) Assess patient on an ongoing basis. Change in patient status (as identified in the Plan of Care) must be communicated to the patient's Physician.

7) Review the patient’s Plan of Care, create 30 day summary of care

8) Clinical notes are to be recorded in Kinnser/Wellsky within 48 hours of patient visit

9) Supervise LVN's and CNA's on an ongoing basis per CMS guidelines

10) Be able to draw lab samples (blood/urine) and deliver to local lab

11) Wound care (including wound vacs) / IV starts & discharges

12) Must agree to comply with the professional license acts or title acts relating to reporting and peer review

13) Monitors and implements physician-ordered treatments including:

a. assess and document patient condition during every visit

b. perform evaluation tasks, including vital signs and medication review

c. administer medication as prescribed by the patient’s Physician

d. wound care educate patients and their families on proper home health care strategies and procedures

14)Coordinate rehabilitation with Occupational Therapists, Physical Therapists, Physicians and all others involved in the patient’s care plan

15) Recommend devices or tools that might improve the patient’s quality of life

16) Listen to the patient and respond to concerns or requests

17) Primarily weekday hours, but will periodically require some weekend support.

Qualifications:

1) Must have a valid Texas State Nursing License (prefer BSN)

2) Must have at least 2 years of experience as an RN

3) Must have at least 1 year of supervisory experience

4) Prefer experience with Kinnser / Wellsky software
