Home Care Field RNs

At Home Solutions is looking for Bi-Lingual fee-for-service Home Care Field RNs for the Syracuse area to provide high-quality patient care in the home. The RN must travel to the patient’s home to administer healthcare services and complete the required assessments.

Conduct ongoing patient care and assessments.

Initiates Plan-of-Care and makes necessary changes as necessary.

Prepares written documentation and summaries of the patient’s status and progress as needed to continue home health services.

Maintain close communication with the Director of Patient Services and the interdisciplinary team.

Responds to phone calls and emails in a timely manner.

Reporting all issues and concerns to the Director of Patient Services immediately.

Completes documentation accurately and submits timely to the agency

Provides the highest quality care in accordance with DOH regulations.

Complete evaluation tasks, including reviewing medication and vital signs.

Administer Physician-prescribed medication.

Dress or redress wounds and assess the healing progress.

Provide education to patients and families on proper home health care procedures and strategies.

Qualifications:

Graduate of an approved school of nursing; Current NYS RN license

Liability insurance is required.

Excellent communication skills, detail-oriented, patient care, and assessment skills

Able to function independently with minimum supervision.

Demonstrates strong critical thinking skills.

Ability to travel to patients’ homes.

Long-term care 1 year preferred.

Nursing home care 1 year preferred.

Rehabilitation center 1 year preferred.

Schedule: Flexible, based on your availability (morning, evenings, and weekends)

Weekly/Monthly caseload - Flexible! You can make your decision

Rate: $100 per plan of care and $50 per HHA Supervision

Salary: $45.00 - $55.00 per hour
